The Public Guardian and Trustee is reviewing the Northern Territory Advance Personal Plan and supporting Guide to make them easier to understand and use.
An Advance Personal Plan can be useful for anyone. If you become sick, injured or are no longer able to make some decisions yourself, it can help your family, doctors and other people supporting you understand what you want.
An Advance Personal Plan lets you:
write down what is important to you
say what you would want to happen in the future if you lose capacity
choose who you trust to make decisions for you if you cannot make them yourself.
We want more people in the community to be able to make an Advance Personal Plan, so the Form and Guide need to be:
clear
simple
easy to complete.
The draft Advance Personal Plan and Guide have been developed following targeted consultation with people who have expertise in this area and people who regularly use or work with Advance Personal Plans.
We would now like your feedback on whether the draft Advance Personal Plan and Guide are easy to understand and use.
Your feedback will help us improve the final versions before they are released.
Please complete the survey below. If you need to provide feedback in a different way please contact pgt@nt.gov.au
