We want to hear from young people, service providers and the community. Let us know what is important to you, what is working well, what needs to change, and how we can work together to make this happen.
The information you give us will be used to further develop the design of the Northern Suburbs Youth Hub. Your information and stories are private and will not be shared without your permission. The questions below are a guide for you to tell us what needs to be considered as part of the design of the Hub, and you can provide other information outside these questions.
This survey will take approximately 15 minutes to complete.

You have been asked to provide personal information necessary for us to be able to contact you in relation to any updates we publish about the status of the Northern Suburbs Youth and Community Hub. You do not have to provide your personal information, but if you choose not to, we will not be able to contact you regarding any key updates.
The information you provide will be accessible to the Department of People, Sport and Culture for the purpose of informing the provision of a department service or program. We will not disclose your personal information to third parties unless:
- authorised or required by law to do so, or
- you have given us your consent to share your personal information for a specific purpose.
