Participants Information Sheet for Parents of Students

Dear Parent/Guardian,
NT Health are conducting an evaluation of the Northern Territory Health Promoting School Nurses Program. We would like to know, are the Health Promoting School Nurses making a difference for your child/ren? Have they helped them make better decisions about their health?
Currently, NT Health has 14 Health Promoting School Nurses who work across the NT in middle, senior and special schools. They have been providing health information to students since 1985.
* Only students with a completed and signed Consent Form can participate in this survey. *
Your child/ren is invited to take part in the Health Promoting School Nurses Evaluation because his/her school has a Health Promoting School Nurse.
The Participant Information Sheet (PIS) lets you and your child/ren know about the project.
Knowing what is involved will help you decide if you want your child/ren to take part in the evaluation. Please read this information carefully. If you don’t understand or want to know more about the project, please contact the research team.
You will be asked to sign an electronic consent form for your child/ren to take part in a survey. If you or your child/ren do not want to participate, you do not have to. At any stage, you or your child/ren can say no by letting either the research team know, or by not completing this consent form.
What is involved?
- You must read this participant information sheet (PIS) carefully, then tick the box stating you have read the sheet and understand it.
- On the consent page, you are required to enter your name, email address and phone number, and your child/ren’s age, year level at school, and nominated email address
- You will be asked whether you consent for your child/ren to participate with two boxes, 'yes' or 'no'.
- Then to add an eSignature which includes your name, location and date; then to press a 'submit' button.
- Your consent will be submitted to the secure email box with the NT Health research project team who will then send an email with the survey link to your child/rens nominated email address so they can undertake the survey.
- Should you or your child/ren withdraw consent after commencement of data analysis, your child/ren’s de-identified information will still be included in the evaluation.
- Before commencing the survey, your child will be asked to insert their age and their year level on the electronic survey form.
- The survey should only take about 30 minutes and will ask your child about 10 questions.
- Their answers will be anonymous. Nil personal information will be collected from the Department of Health or the Department of Education.
What happens when the review project ends?
The evaluation will be completed by the end of 2023. A report will be provided to the Minister for Health and the Minister for Education as this is an election commitment. Each school principal will be provided with the evaluation findings, and a one-page flyer detailing the findings for distribution through the respective school newsletter/bulletin.
Who has reviewed the research project?
This project is approved by the Human Research Ethics Committee of NT Health and Menzies School of Health Research, as required by the Australian Government research requirements, specified in the National Statement on Ethical Conduct in Human Research (2018). This statement has been developed to protect the interests of people who agree to participate in human research studies.
